MississippiCare’s Remote Patient Monitoring program is a 12-week program for patients with uncontrolled hypertension or those who have had a recent change in hypertensive medications. You are provided with a blood pressure monitor that transmits readings to a secured HIPAA compliant portal from the comfort of your home. A nurse will monitor these readings and provide you with education on how to lower your blood pressure. The results from the program will be shared with your healthcare provider and team.
